Project Description: 

THP-Plus is a Transitional Housing Placement-Plus Program that assists emancipated young adults as they move from dependency to self-sufficiency by providing housing and supportive services. In additon, THP-Plus funds from the Housing and Community Development will be utilized to help young adults 18-25 secure and maintain housing. The Transitional Housing Program (THP) and Housing Navigation Program (HNP) funds will be utilized to support young adults who are current or former foster youth, juvenile justice involved youth, and youth experiencing homelessness who are between the ages of 18-21.

Description of Services:

THP-Plus program will provide preventive and supportive services to the targeted youth to help them reach their short-term and long-term goals. Eligible youth will receive Case Management services, 24-Hour crisis intervention and support, individual and group therapy (directly or through a referral), education advocacy and support, assistance in pursuing college or other post-high school training, mentoring, and life skills training. Additional services may include economic literacy training, transportation assistance, security deposit assistance, and aftercare services including referrals to community resources. 

Program Goals:

Goal 1: THP-Plus program will assist emancipated foster care youth to secure stable housing. 

Goal 2: THP-Plus program will increase the employability of emancipated foster youth. 

Goal 3: THP-Plus program will encourage emancipated youth to further their educational goals. 

Goal 4: THP-Plus participants will be enrolled in Medi-Cal or other private/public health insurance program. 

Eligibility/Targeted Participants:

THP-Plus serves young adults who are at least 18 years of age and not yet 24 years of age. Youth can be up to 25 years of age if they are enrolled in a post-secondary institution. 

Funder:

Funding is provided by Imperial County Department of Social Services- Child Welfare Services.

Length of Funding:

Funding is provided on an annual basis.
