6. Are you aware that if you fail or drop enough classes after the deadline you might be placed on “academic probation”? Are you also aware that you will not receive financial aid, scholarships or the Chafee Grant if you are placed on “academic probation”?

7. Did you have trouble buying books and supplies for your classes the first 2 weeks of class? (If so, please explain)

8. Have you experienced any obstacles or problems that might prevent you from completing your classes? If so, what problems?

Excellent (No problems. I should complete my education soon.)

Fair (Some problems but I should complete in 2 years. Help would be good.)

Poor (Lots of problems. I will have trouble completing. I can use help.)

Horrible (I was not prepared financially or academically. Please help!)
